Remind me on the spot

I would like to have an app that reminds me what I need to/should do when I’m nearby a specific place, a Location-based-reminder.
The reminder should be triggered by the position or position and timeframe.
Let say that you are running out of pain killers and need to remember that next time you’re in town or nearby the pharmacy you should be reminded to buy more.
Or a reminder that you should pick up your kids when you’re passing kinder garden. (I know people that forget that :)
Or you have received a voucher that you want to use but you never remember to use it when you are nearby or at the store during the valid period. (There is probably a market for mobile vouchers that includes a position based reminder.)
Or the salesmen that need to remember that he shall speak to Gary the next time he visits customer X or visit both company y and z when he is in the area.
Mobile positioning without GPS should work since a reminder when you are in the area should be good enough in most cases.
The advantage of net based mobile positioning is that it works as soon as the phone is turned on and connected to the net so it works on all mobile phones.
Cons: The cost to continuously ask for your net based mobile positioning is rather high today. But the mobile also always knows the current cell id but then you need to be or have been in the cell to know the name or id when you shall set the reminder. This is due to the fact that the operators seldom want to distribute a list with the cell id’s and their positions.
GPS positioning would of course also work if always turned on.
An observation is that patents seams to exist in this area (but which corner isn’t patent registered today)
